Who is an eligible person for our Services?
We provide services in two funded programs:
Queensland Government – Home Assist Secure program.
Federal Government – Community Home Support Program.
See below to determine which program suits your specific situation:
Are you?
- 60 years and over, or
- Any age with a disability
- Are a home owner or living in rental housing
You are eligible for the Home Assist Secure program – free information and referrals only.
Are you?
- 60 years and over, or
- Any age with a disability
- Are a home owner or living in rental housing
- Hold a current Pensioner Concession Card
- Do not possess the appropriate skills to undertake the work or whose health or safety would be at risk if they undertook the work yourself,
- Are unable to make use of alternative forms of assistance, such as Home and Community Care, Community Care Services (qld), Department of Veteran’s Affairs, family or friends.
You are eligible for the Home Assist Secure program – free information and referrals, PLUS subsidised assistance for work in your home.
Are you?
- 65 years and older (or 50 years and older for Aboriginal and Torres Strait Island people)
- Live in the community ie. Not in a residential care facility
- Having difficulties performing the tasks of daily living and need assistance or supervision due to ongoing moderate, severe or profound functional disability; and without this assistance would be at risk of admission to long-term residential care
- Tasks of daily living include dressing, preparing meals, house cleaning, maintenance and using public transport.
You may be eligible to receive assistance under the Community Home Support Program (CHSP).
You must contact My Aged Care on 1800 200 422 or through the website www.myagedcare.gov.au to be assessed and start receiving services.
When you call My Aged Care tell them that Sandgate Home Assist is your preferred provider of home maintenance and modifications. You will then receive a referral code for maintenance and modification services. Please call us and give us these codes, we can start helping immediately!
