If you think you may be eligibile for Home Assist Secure or Commonwealth Home Support Program please call our office on 3869 1000 and we will provide advice on how you can become registered with us.  Alternatively please complete the below form and one of our staff members will be in touch.

Please note the following information is obtained for the primary purpose of Sandgate Home Assist contacting you to discuss your eligibility to participate in our Service.  Further information about this can be obtained by speaking with the Manager on 3869 1000.
